Introducing a new website for complex care

On behalf of the National Center for Complex Health and Social Needs, I’d like to introduce you to our new digital home, NationalComplex.Care.

The new site is designed to facilitate the creation of evidence-based models for improving the care for individuals with complex health and social needs. Built with busy practitioners and providers in mind, it allows you to:

  • Stay current with curated resources and best practices from the field.

  • Learn from leaders with our Trailblazer profiles; featuring innovators like Kallol Mahata and Jim Withers.

  • Help grow your programs with compelling economic arguments for treating individuals with complex health needs.

  • Connect with the National Center for technical assistance and personalized trainings.
