Brian Akers: Program Manager, Production Management

Brian Akers joined the Camden Coalition of Healthcare Provides in 2012 as an AmeriCorps Volunteer Healthcoach. After his year of service, he assisted as the Interim Office Manager. In 2015, he transition to the Cross-Site Learning and Workforce Development team, where he helps with content and research curation. Brian received his Bachelor of Science from the Pennsylvania State University.

Gladys Antelo: Senior Nurse Care Coordinator, Innovation Operations

Gladys Antelo is a senior nurse care coordinator for Care Management Initiatives. She works closely with primary care practices, specialist offices and Coalition staff to improve healthcare access and provide evidence-based chronic disease care for Camden city residents. A resident of Camden, Gladys is passionate about empowering Camden city residents to engage in healthy behaviors. Gladys received her Practical Nursing Diploma from the Lincoln Technical Institute.

Soley Berrios: Program Assistant, Policy & Engagement

Soley is the Program Assistant for Policy & Engagement in the Legal and External Affairs Department at the Camden Coalition. She graduated from Rowan University where she studied Radio/TV/Film Productions with a concentration in New Media. Soley spent the last year working with UrbanPromise Ministries, a dynamic not-for-profit in Camden City, as a Community Organizer for the Delaware Gardens Neighborhood Initiative. She played an instrumental role in launching a Community Youth Council and Neighborhood Group.

Adria Boultin: Program Assistant, Information Systems & Data Security

Adria is a Program Assistant for Strategy & Information team; her role is implementing strategy and policies within the Camden Coalition. Adria is currently studying Computer Science at Camden County College and hopes to transfer to Temple University. In her free time she loves to volunteer in community-based programs that prevent homelessness.

Jeffrey Brenner: Chief Executive Officer

Jeffrey Brenner, M.D. is a family physician that has worked in Camden, NJ for the past fifteen years. Dr Brenner owned and operated a solo-practice, urban family medicine office that provided full-spectrum family health services to a largely Hispanic, Medicaid population including delivering babies, caring for children and adults, and doing home visits. Recognizing the need for a new way for hospitals, providers, and community residents to collaborate he founded and has served as the Chief Executive Officer of the Camden Coalition of Healthcare Providers since 2003. Through the Camden Coalition, local stakeholders are working to build an integrated, health delivery model to provide better care for Camden City residents. Dr. Brenner’s work was profiled by the writer and surgeon Dr. Atul Gawande in an article in The New Yorker entitled “The Hot Spotters” (1/24/11) and in an episode of PBS Frontline (7/27/11). In 2013 he received a MacArthur award. Dr Brenner is the Medical Director of the Urban Health Institute, a dedicated business unit built at the Cooper Health System focused on improving care of the underserved. Using modern business techniques they are redesigning long-standing clinical care models to deliver better care at lower cost.

Sheila Brown: CFO & Senior Director of Administration

Sheila Brown is the CFO & Senior Director of Administration. Prior to joining the Coalition, she worked as the Controller for Merit Service Solutions. She spent 10 years working as a public accountant with the firm Withum Smith+Brown specializing in auditing and consulting for not for profit organizations. Six of those years were spent working primarily on the Healthcare Team. She was Director of Finance for Ambulatory Services at Virtua, and Controller for Silver Labs LLC,

Whitney Buchmann: Program Manager, Organizational Effectiveness

Whitney is the Program Manager for Organizational Effectiveness, whose primary role is to support the development of processes and practices that keep the Coalition's core values at the center. Prior to coming to the Coalition, she spent six years as a Community Organizer with PICO National Network, the last three of which were with the Camden affiliate. Whitney holds a bachelor's degree in Spanish and International Studies, with a certificate in Conflict Resolution.

Laura Buckley: Senior Program Manager, Social Work Operations

Laura Buckley, M.S.W., L.S.W., leads a team of clinical specialists to design, iterate, and operationalize interventions that impact individuals with complex health and social needs, most notably overseeing the daily implementation of the Camden Coalition's Housing First Demonstration Project. Prior to joining the Coalition, Laura was a graduate teaching assistant at the University of Pennsylvania's School of Social Policy and Practice working in both Philadelphia and Kolkata, India. She received her master's degree in social work from the University of Pennsylvania with a clinical concentration and her Bachelor of Arts from New York University with a double major in history and journalism.

Gayle Christiansen: Program Manager, Content Management

Gayle creates curriculum and learning experiences from a human-centered design perspective at the Camden Coalition. She has experience teaching and designing instruction for elementary school through late career professionals. Gayle first came to Camden as a Camden Public Schools teacher. Gayle studied Sociology as an undergraduate at Kenyon College and holds a Master of City Planning degree from MIT. Gayle also serves as a part-time lecturer in the Urban Studies Department at Rutgers University – Camden.

SENIOR COMMUNITY HEALTH WORKER

Jessica Cordero: Senior Community Health Worker, Community Operations

Jessica Cordero is the medical assistant and senior community health worker for the Coalition’s Care Management intervention. With several years of experience managing a busy community-based primary care practice in Camden, she currently uses the same skills coordinating care for the project’s high needs patients. Additionally, she provides various levels of clinical assistance on site during home visits, as well as a liaison for patients who are Spanish-speaking. In serving patients who are medically and socially complex, she has become a dependable and trusted resource for these city residents. Jessica grew up in Camden and earned a degree as a Medical Assistant in New Jersey.

Kelly Craig: Chief Strategy & Information Officer

As one of the original ten members of the Camden Coalition, Kelly worked closely with Executive Director, Jeffrey Brenner, M.D., in developing the Coalition’s innovative care management model. This model engages patients with multiple co-morbidities that lead to high utilization of the local healthcare system. Kelly implemented the first managed care contract that lead to the Coalition's current Medicaid Accountable Care work. Additionally, she grew the Camden citywide Care Management Meetings that bring together stakeholders from three competing hospitals around common community barriers and shared patients. The structure she built over five years still meets monthly and serves as an anchor of collaboration within the Camden community.

Prior to coming to the Coalition, Kelly worked at Prevention Point Philadelphia, a harm reduction organization. There she coordinated the Street-side Health Project, a mobile medical clinic run solely on volunteer labor in partnership with local medical, nursing and pharmacy schools. Her passion drove her to establish the first low-threshold buprenorphine clinic in the area, giving access to individuals experiencing homelessness and opiod addiction. These programs are still thriving in Philadelphia. Currently, Kelly is focused on supporting the design, development, and expansion of a variety of population health initiatives taking place at the Coalition. Some current initiatives include overseeing our citywide 7-day pledge campaign, which focuses on getting patients into primary care within 7 days of a hospital visit. She is also working with community partners to advance the first regional Housing First program, a low-threshold model that provides housing and supportive services to the most medically fragile patients within Camden.

Amadly Cruz is a program assistant for the Care Management Initiatives team. She received her B.A. in Psychology at Eastern University and plans to go back for her M.S.W. Born, raised and currently living in Camden, Amadly feels a deep connection with the community and wants to influence positive health to the people through the help of the Camden Coalition.

Meylin Davila: Program Assistant, Clinical Redesign Initiatives

Meylin Davila is the program assistant for the Clinical Redesign Initiatives team. As the program assistant, she holds a B.A. in Sociology from Rowan University. Being in this role, Meylin hopes to have a great influence on the community and spread the word on all the great things the coalition is doing. Currently, Meylin is the Diabetes Self-Management Education project assistant for the Camden Coalition.

Sophia Demuynck: Research Coordinator, Action Research & Evaluation

Sophia serves as the Research Coordinator for the Action Research & Evaluation department, where she provides organization and management of knowledge generated by and relevant to diverse initiatives at the Camden Coalition. She joined the Coalition in 2013 as Program Assistant for the Camden HIE and has a background in clinical and population health research. Sophia holds a BA in Sociology and Neuroscience from Smith College and completed a post-baccalaureate program at the University of Pennsylvania.

Seema Doshi: Health Coach

Seema Doshi is serving as a Health Coach with Project H.O.P.E. through the Camden Coalition. She graduated from Haverford College where she studied Biology and Social Justice. Prior to joining the Coalition, she worked at Puentes de Salud, a nonprofit in South Philadelphia. In her free time she enjoys cooking and being outdoors.

Natasha Dravid: Associate Director, Business Planning & Continuous Improvement

Natasha Dravid joined the Camden Coalition in July of 2013 and serves as Associate Director for Business Systems & Continuous Improvement on the Strategy & Information team, where she focuses on leading efforts to build the capacity of the organization to design, scope, launch, manage and evaluate projects aimed to achieve the strategies of the organization. Prior to joining the Strategy & Information team, Natasha served as senior manager for Clinical Redesign Initiatives where she focused on standing up the infrastructure for the Camden Coalition Medicaid Accountable Care Organization and quality improvement within primary care practices in Camden. Natasha holds an M.B.A. from the Yale School of Management and a B.A. in English from Haverford College.

Molly Ennis: Program Assistant, Policy & Engagement

Molly Ennis is a Program Assistant for the Legal and External Affairs Department, focusing on Policy and Research. Previously, Molly worked in Louisiana at the New Orleans Health Department, and later as an Evaluation and Research Assistant for an arts integration program that aims to improve cognitive development in preschool students. She also worked in health policy at the Alliance for Health Reform in Washington, DC. Molly earned a BA in Environmental Studies from the University of Michigan and a Master’s Degree in Public Health from Tulane University School of Public Health and Tropical Medicine.

Abigail Fallen: Senior Program Manager, Health Information Exchange & Data Security

Abby Fallen is the program manager for the Camden Coalition Health Information Exchange (HIE). Abby oversees the program’s objectives while working with Camden’s area hospitals and primary care centers to develop effective training processes. Before joining the Coalition, Abby worked as an EMR Training Specialist for STI Computer Services, aiding and assisting various physician practices in transitioning their medical records from paper to an electronic format. Additionally, Abby has extensive knowledge of office workflows, clinical medicine, inpatient and outpatient documentation, and systems analysis. Abby is a graduate of Temple University and holds her Registered Health Information Administrator (RHIA) certification.

James Fisher: Program Manager, Data Integration

Jay is the program manager for the Information & Systems team at the Camden Coalition. He recently worked as a statistical research specialist for TennCare, Tennessee's Medicaid office. He also served for two years as a community health volunteer in Cameroon with the Peace Corps. Jay received his bachelor's degree in International Relations from the College of Wooster and his master's degree in Public Administration from the University of Georgia. Jay is interested in Medicaid policy, running when he can, and watching basketball.

Marizta Gomez: Senior Community Health Worker, Community Operations

Maritza Gomez is a senior community health worker of the Camden Coalition of Healthcare Provider’s Care Management Initiatives. Maritza’s current role requires extensive work with patients in the community, where she provides clinical assistance and support to patients with the goal of meeting their health and social needs. Prior to joining the Camden Coalition Maritza has many years of experience managing a local business. As a community health worker she uses those skills of high level engagement and being responsive to persons in need to advocate on behalf of community residents. Maritza is passionate about serving as a liaison and advocate for Spanish speaking patients in the community. Currently Maritza attends Burlington County College earning a degree in nursing.

Ebony Hailey: LPN Care Coordinator, Community Operations

Ebony Hailey is a licensed practical nurse care coordinator on the CMI team. Ebony works closely in the community, bridging the gap between patients and their healthcare providers. She was born and raised in the City of Camden and has a love for giving back to the community and mentoring. Prior to joining our team, Ebony was a Critical Care Technician in the Intensive Care Unit at Cooper Hospital University Center for seven years. She is currently pursuing her B.S.N.

Lauran Hardin: Senior Director, Cross Continuum Transformation

Lauran Hardin recently joined the Camden Coalition and the National Center for Complex Health and Social Needs as the Senior Director of Cross Continuum Transformation. In this role she continues her past experience providing consulting, co-design and coaching in complex care transformation. Her special interests include the impact of trauma/loss on high frequency healthcare access and the economic potential of stabilizing complex patients through retraining/redesigning existing resources in the healthcare system.

Kaiya Harris: Program Manager, Community Engagement

Kaiya works as the Program Manager for Community Engagement on the Legal and External Affairs Department. In this role Kaiya uses her extensive knowledge of the primary care landscape in Camden and the day-to-day operations of the Camden Coalition ACO, to help expand the Camden Coalition's civic engagement work and integrate the community engagement work into our ACO strategies. Kaiya earned a degree in Public Health from Richard Stockton College before serving as an urban middle school science teacher for ten years and working in nonprofit program development and administration for over five years. While her greatest passion is engaging in faith-based social justice advocacy, she also enjoys working toward community empowerment through adult literacy and education in the city of Camden and being “Auntie Kaiya” to her awesome nieces and nephews.

Margaret Hawthorne: Director, Strategy & Innovation

Margaret Hawthorne serves as the director of strategy and innovation for the Camden Coalition, where she works with diverse stakeholder groups to design, evaluate, and transform operational processes and care delivery systems. Prior to joining the Coalition, Maggie worked as a Program Officer with the Institute of Medicine evaluating U.S. global HIV/AIDS programs for the US Congress, an epidemiologist managing the operations for two statewide HIV surveillance systems, and a Research Manager at Johns Hopkins University focused on developing tobacco control leadership capacity globally. Margaret holds a master’s degree in Public Health from Johns Hopkins University and is passionate about employing a mix of both qualitative and quantitative methods to understand, design, implement, and evolve relationships and systems that will lead to profound impact and change.

Audrey Hendricks: Enrollment Specialist, Hospital Operations

Audrey Hendricks is an enrollment specialist on the hospital operations team for Care Management Initiatives. In this role, she serves as the first point of contact for hospitalized patients who are eligible for Camden Coalition's Care Management Intervention, engaging and enrolling them into the intervention at the bedside. Audrey joined the Camden Coalition in 2012 as an AmeriCorps Health Coach. Following her year of service, she worked as a program assistant in the Cross-Site Learning program before beginning her current role in early 2015. She holds a B.A. in Anthropology from Haverford College.

Mark Humowiecki: General Counsel & Senior Director for National Initiatives

Mark Humowiecki leads the National Center for Complex Health and Social Needs, a new initiative of the Coalition that seeks to coalesce a new field of healthcare and build a national movement to improve outcomes for individuals with the most complex needs. Previously Mark led the Coalition's advocacy, communications, and governance efforts. Before joining the Coalition, Mark served as deputy executive director of the New York State Workers' Compensation Board and spent six years practicing employment and civil rights law. Mark earned his bachelors and law degrees from Yale University.

Jared Hunter: Program Assistant, Innovation Operations

Jared Hunter serves as the Program Assistant of Innovative Operations at the Coalition. Jared works within Population Health Initiatives to envision, invent, and implement cutting-edge strategies to improve certain populations. He recently graduated from the University of the Sciences with a Bachelor of Science in Humanities and Science. He is currently attending Rutgers-Camden to pursue a Master of Public Administration with a concentration in Community Development. Jared's major passions include utilizing community development, public policy, and politics to find new and progressive ways to improve the lives of his surrounding communities.

Stephanie Jean-Louis: Program Assistant, Stakeholder Relations

Stephanie Jean-Louis is the Program Assistant for Stakeholder Relations. In this role she coordinates communication both externally, to key stakeholders and community partners, and internally. She received her Bachelor's degree in Biobehavioral Health from Penn State University. She is passionate about community organizing and enjoys spending time with family and friends.

Marsha Johnson: Associate Director, Cross-Site Learning & Workforce Development

Marsha is the Associate Director of Educational Programs at the Camden Coalition. Her work focuses on the dissemination of the Hotspotting philosophy to the broader field of population health, and training the workforce in the tools and mindsets necessary to transform health care delivery. A licensed clinical social worker, Marsha received her master’s degree from Smith College School for Social Work and spent that last fifteen years in healthcare delivery and design for complex patients.

Naadia Johnson: Program Assistant, Organizational Operations

Naadia is a Rutgers University graduate with a B.A. in psychology and now fills the role of program assistant for Camden Coalition. As her favorite core value is servant leadership, her objective is to offer continued organization to better help our staff empower our patients and members. Future goals include continued education in social work with plans to provide services that give back to the city of Camden, her free time is spent reading, taking trips and scrap booking for memories.

Teray Johnson: ACO Outreach Clerk

Teray Johnson works on the Clinical Redesign Initiatives team as an ACO Outreach Clerk. In this role she schedules patient PCP appointments and provides education on 7-day post-hospital follow-up visits. Teray has a Bachelor's of Science in Healthcare Management from Towson University and is currently pursuing an MBA with a specialization in Management Information Systems from Rowan University. She is passionate about writing stories, mostly fantasy, and making sure all patients are treated the way they would like their own loved ones treated.

Andrew Katz: Senior Program Manager, Care Management Initiatives

Andrew Katz is a senior program manager of Care Management Initiatives and focuses on operations and care delivery for one of the Coalition's outreach teams. Andrew joined the Coalition in 2011 as an AmeriCorps health coach and has since worked on CCHP projects like Diabetes Self Management Education, Integrated Diabetes Care Program, and practice transformation at two primary care clinics in the City of Camden. Prior to joining the Coalition, Andrew studied Psychology at McGill University.

Samuel Katz: Program Manager, Business Planning & Continuous Improvement

Samuel Katz is the Program Manager for Business Planning & Continuous Improvement at the Camden Coalition. In this role he helps implement, monitor, and evaluate the Coalition's strategic initiatives. Previously, Samuel worked at the Mixed Methods Research Lab at the University of Pennsylvania. He has also worked as a medical-legal writer for a disability advocacy group, an EMT, and an assistant to a gourmet chef. Sam earned a B.A. from Wesleyan University and is currently pursuing an M.B.A. at Temple University.

Angela Klanchar: Health Coach

Angela is a Health Coach at the Urban Health Institute. She studied Psychology at Rowan University and her most notable experience as a volunteer includes working at a mobile clinic in Lima, Peru. Angela loves yoga and music, especially alternative rock bands such as The Vaccines and Arctic Monkeys.

Mary Kleva: Program Manager, Stakeholder Relations

Mary Kleva is the Program Manager for Stakeholder Relations within the Legal and External Affairs Department. She works on grant reporting and development, advocacy and policy initiatives, Camden Coalition board activities, and additional communications for the department. Prior to joining the Coalition, Mary was a research assistant at Public Health Management Corporation in Philadelphia. Mary graduated from the University of Pennsylvania with a B.A. in Health and Societies, concentrating in Health Policy and Law.

Kurt works as a Health Coach serving with the Addiction Medicine Program at the Urban Health Institute, helping patients with substance use disorders improve their well being and navigate community resources. He received his bachelor’s degree in Health and Societies from the University of Pennsylvania. Kurt is passionate about working with underserved populations to address disparities in health care access and plans to attend medical school after his year of service with the Coalition.

Teagan Kuruna: Research Writer, Action Research & Evaluation

Teagan Kuruna serves as the Research Writer for the Action Research and Evaluation department, where she develops written content for journal articles, reports, and other publications. Prior to joining the Camden Coalition, she was a communications associate at the Center for Health Care Strategies and a communications intern for The Cancer Genome Atlas at the National Cancer Institute. Teagan holds a master of public health from Drexel University School of Public Health and a bachelor of arts from New College of Florida. In her spare time, Teagan organizes the Philadelphia Podcast Festival and hosts her own podcast.

Berly Laycox: Program Manager, Communications & Digital Strategy

Berly Laycox is the Program Manager for Communications & Digital Strategy at the Camden Coalition. Berly has a B.A. in Communication from UMass Amherst and a B.F.A. in Print Media from Highlands University in Las Vegas, NM. In her spare time she works as the New Jersey chapter leader for Girl Develop It.

Angelic Lopez: Accounting Manager, Accounting & Grants Administration

Angelic has joined the Camden Coalition as an Accounting Manager where she is responsible for managing the accounting team to ensure transactions are recorded accurately and timely to meet the organization's deadlines. Angelic received her Bachelor of Business Administration from Peirce College and her Master of Business Administration from Strayer University. Angelic likes to spend her free time with her family and friends - she enjoys helping others.

Zachary Martinez: Program Assistant, Analytics & Informatics

Zachary supports the Camden Coalition's patient and provider facing teams with data analyses and performance management tools. Prior to joining the Coalition, Zachary graduated from Dartmouth College with a B.A. in Biology and concentration in Neurobiology and Behavior. Zachary also co-led a trail crew on the Appalachian Trail, and interned with Boston Healthcare for the Homeless Program. He is currently pursing an M.S. in Business Analytics at Temple University's Fox School of Business.

Marah is a Super-utilizer Fellow with Crozer Keystone for the 2016 - 2017 program year. Marah provides clinical expertise for the Cross-Site Learning department, specifically with Student Hotspotting program. Marah graduated from residency in Family Medicine up in Allentown, PA. She loves being back in Philadelphia where she finally gets to live with her husband and dog named Stinky.

Christine McBride is the program manager of Health Information Exchange (HIE) & Data Security. Christine's role is to plan and manage projects for new HIE interfaces and functionality, ensures compliance with the HIE Policies and Procedures through auditing and education, and assists in data security initiatives related to the approval, implementation and staff training of policies and procedures. Prior to joining the Coalition, she worked for a consulting firm as a marketing administrator and a project consultant. Christine graduated from The University of Scranton in 2013 with a bachelor's degree in Communication and minors in Spanish and Philosophy.

Tynnetta McCain is the senior program manager for Human Resources (HR) at the Camden Coalition where she is responsible for HR strategies, policies and procedures, talent management, staff development, benefits and general office administration. Tynnetta received her bachelor's degree in Organizational Management from Eastern University. In her free time, she enjoys reading, exercising, and watching sports (football and tennis).

Hannah Mogul-Adlin is the program assistant for Communications & Stakeholder Relations in the Legal and External Affairs Department, working on internal and external communication, social media, blog content, and photography for the organization. Prior to joining the Coalition, Hannah worked at Planned Parenthood Southeastern PA as a clinical research coordinator. She holds a B.A. in Anthropology and a Master of Public Health from Yale University. In her free time, Hannah organizes Philly Queer Brunch, a monthly community-building initiative that fund raises for local and national justice-oriented organizations.

Dani Mooney is the Program Manager for Online Education for the Cross-Site Learning Department. Her other interests include emergency preparedness, reading (mostly nonfiction), and yoga.

Suzanne Morgan's role provides high-level organizational support and works to improve communications and processes across the Coalition. A graduate of Duke University, Suzanne was previously an Emmy-award winning producer for NFL Films. An artist and arts educator, in her free time Suzanne continues inspire students and create her own art ranging from paintings, photography and sculpture to large-scale mosaics and murals.

Renee Murray: Associate Clinical Director, Care Management Initiatives

Renee Murray graduated from the University of Medicine and Dentistry of New Jersey with a B.S.N. and previously received a B.A. in Psychology from St. Joseph’s University. Her prior experiences include teaching at Camden Catholic High School instructing Psychology, Health, and Cultural Geography courses as well as being the Admissions Director. She also spent some time in the Dominican Republic setting up makeshift clinics to provide healthcare to the underserved locals. Most recently she practiced nursing on an Oncology/Hematology unit at Hahnemann University Hospital in Philadelphia. She joined the Coalition in 2012 as the RN Care Coordinator for the Pregnancy and Parenting Partners Program working with pregnant women throughout the city of Camden. Renee is now the RN Clinical Manager on Care Management Initiatives. CMI is a nationally recognized clinical intervention and research project seeking to improve health outcomes for complex, vulnerable patients through intensive community-based care management and care coordination activities.

Victor Murray serves as director for Population Health Initiatives. Victor focuses on working across the Camden Coalition to design, iterate and operationalize new data-driven interventions that will impact specific segments of the population. Prior to this role, he formed a hospital-based violence intervention program and served as program manager for Camden Coalition’s community-based outreach team. Victor enjoys playing a role in providing better care to Camden City residents and living life.

Bill Nice: Social Work Coordinator, Social Work Operations

Bill Nice is the Social Work Coordinator for Care Management Initiatives. His primary role involves working directly with patients who are homeless or at risk for homelessness, connecting them to local service organizations and community resources while also ensuring they have an advocate who can help them navigate the social service system when necessary. Bill is a Communications graduate of Rowan University and is a current Rutgers Social Work Masters student. Over the last two and half years Bill has worked with Camden’s homeless and will be undertaking a nearly yearlong placement through Rutgers in one of Camden’s drug and alcohol programs to learn about addictions.

Ramona Olivares: Program Assistant, Clinical Redesign Initiatives

Ramona Olivares is the program assistant for the Clinical Redesign Initiatives, where she assists the certified diabetes educators in all aspects of diabetes education implementation including referral and registration, in-class support, patient data collection, and information sharing with primary care physicians. She also serves as a point of contact between collaborative staff. While working at River Road Primary Care in Camden, Ramona worked closely with the Coalition helping to implement the Spring Chart electronic medical record system. She is a graduate of the Harris School of Business with a professional medical assistant certificate.

Latonya Oliver: Social Work Coordinator, Social Work Operations

Latonya Oliver, M.S.W., is the social work coordinator for the Care Management Initiatives - Social Work Operations Team. Latonya works primarily with marginalized and underserved populations within the city of Camden and assists with support, planning, coordination and advocacy as needed to improve the quality of life and care for CCHP’s most complex patients. Latonya graduated from Stockton University with a B.A. in Psychology and a minor in geriatrics. Latonya has recently graduated from Rutgers University- Camden campus in May 2014 with a Masters of Social Work and a concentration in clinical social work. During graduate school Latonya also received a certification in aging/geriatrics. Previously, Latonya has worked with severe and persistently mentally ill clients within the Camden County area; during this time she was responsible for monitoring and using various interventions and techniques to reduce client readmissions to state and local psychiatric hospitals. Latonya also has work experience as an in-home therapist working with children who demonstrate emotional and behavioral issues due to exposure to trauma. Latonya hopes to expand her professional career and become a licensed clinical social worker.

Prior to working with the Coalition Marcus has worked in several health related fields in South Jersey including in-patient medical, psychiatric and community healthcare delivery. Karen Rentas: Behavioral Health Consultant, Care Management Initiatives

Karen Rentas, Ph.D is the behavioral health consultant for the Coalition through Rutgers University Behavioral Health Care. She plays a leadership role in developing and delivering staff training and evaluating and coordinating care for patients with significant behavioral health issues. Prior to joining the Coalition she worked as a Mental Health Clinician for Rutgers University Behavioral Health Care at the New Brunswick Outpatient office providing individual, group and family therapy to adolescents and adults with mental illness, trauma and issues of addiction. Dr. Rentas also has experience providing juvenile sex-offender specific treatment and collaborating with research. Karen is committed to work with underserved populations and to reducing health disparities. She received her doctoral degree in Clinical Psychology from Carlos Albizu University in San Juan, Puerto Rico.

She previously served as Law Clerk to the Honorable Robert G. Millenky, Presiding Judge, Superior Court, Civil Division, Camden County. She previously worked as program manager for Research, Data and Evaluation for the Montgomery County, Maryland, Department of Health and Human Services, and held positions at the Center for Medical Technology Policy and The Lewin Group. Emily holds a bachelor’s degree in policy analysis and management from Cornell University and a Master of Science degree in public health from the Johns Hopkins Bloomberg School of Public Health.

Natassia Rozario oversees policy and advocacy efforts at the Coalition. She was a recipient of the Public Interest Public Service Scholarship at American University Washington College of Law and of the William J. Clinton Service Fellowship in India, and brings to the Coalition over ten years of experience and research devoted to improving the availability, accessibility, acceptability, and quality of healthcare for the underserved both in the US and abroad. She has a JD cum laude from American University Washington College of Law, a master's degree in Public Health from Johns Hopkins University, and a bachelor's degree from Columbia University, where she majored in political science. She received a Fulbright-Nehru research fellowship to study health and human rights in India and is a member of the New Jersey and New York Bar Associations.

Victoria oversees the Camden Coalition's education efforts from local medical training to national technical assistance. Shabnam Salih, as senior program manager for State Engagement, helps lead the Good Care Collaborative, a statewide coalition for NJ Medicaid reform. Stephen is interested in data-informed community organizing and institutional design. Prior to joining the Coalition, Stephen worked as a Data Specialist with Benefits Data Trust, the Executive Director at CamConnect (Camden’s non-profit data warehouse and civic research organization), and the Senior Research Manager at the Philadelphia Center City District. Stephen holds a Master of City Planning from the University of Pennsylvania and B.A. in Population Studies and Social Problem Solving from Rutgers University.

Jeneen Skinner: Senior Nurse Care Coordinator, Community Operations

Jeneen Skinner is a licensed practical nurse care with the Care Management Initiatives team. She serves as the the primary liaison between patients, health care providers and community organizations to address complex health and social needs in an effort to reduce hospital and ED utilization. She brings with her experience in home care, employee health/infection control, and community outreach to the city of Camden’s homeless and under-served populations. Jeneen is currently pursuing her RN degree at Burlington County College.

Jared Susco: Chief Operating Officer

Jared grew up in Syracuse, NY but quickly fell in love with the Philadelphia/Camden area while an undergraduate at the University of Pennsylvania. He graduated from the Wharton School as a Bachelor of Science in economics with a concentration in accounting but decided to work in marketing strategy on Cap’n Crunch cereal and Quaker rice cakes for PepsiCo in Chicago. He then served as a rural economic development volunteer with the Peace Corps in Paraguay and subsequently pursued a certificate in Hebrew and Jewish Studies from the WUJS Institute in Israel. After returning to the US to earn a master’s degree in business administration with a concentration in decision and information analysis from the Goizueta Business School of Emory University, Jared moved back to the Philadelphia/Camden area to work in brand management with the Campbell Soup Company on several soup businesses.

Jared took a position as the director of operations and planning with the Undergraduate Division of the Wharton School, where his love began for strengthening operational structure and organizational capacity in the mission-based space. With successful stints as the comptroller and then chief financial officer of Penn Dental Medicine, he fully decided that operations and leadership in community-based healthcare was where he wanted to be. He is very excited to serve as the chief operating officer of CCHP, where he brings business principles, like continuous improvement, systems thinking, and project planning, to the effective and efficient execution of the mission. Jared’s focus is largely internal, with emphasis on deepening structure, metrics, process, analysis, and planning for CCHP’s existing programs and on supporting outcome- and performance-driven teams with mentoring, coaching, coordination, and integration. Jared and his husband live in the Society Hill area of Philadelphia, where they are slowly restoring the 204-year-old row home in which they live. Outside of work, Jared enjoys traveling, hiking, singing, supporting the arts, and spoiling his nieces and nephews.

Summer Tatum: Program Manager, National Initiatives

Prior to joining the Camden Coalition, Summer served as the Special Assistant to the Higher Education Advisor for the City of Philadelphia. While in the Mayor's Office of Education, Summer contributed to the design and implementation of initiatives that expanded access to higher education programs and services for all Philadelphians. Since joining the Camden Coalition, Summer has continued the fight to improve access and quality as the Program Manager for Community Engagement, deepening local relations and implementing city-wide public health initiatives like Faith in Prevention, which expands the role of the faith community in the delivery of health promotion services. Summer now serves as the Program Manager for National Initiatives, uplifting the local voice to a national platform, and seeking to help to improve the lives of individuals with complex health and social needs.

Aaron Truchil: Director, Strategy & Analytics

Aaron Truchil serves as the Director for Strategy & Analytics at the Camden Coalition, where he oversees the organization’s data and research activities, including the Coalition's integrated data system that merges individual-level data from across sectors (health, criminal justice, and other social services), the Coalition's shared savings performance evaluations, and a variety of applied health services research initiatives aimed at developing an evidence base around community-based care and cost management initiatives. Prior to joining the Coalition, Aaron worked as a program manager at CamConnect, a non-profit data warehouse in Camden that analyzes and reports on data on the City of Camden’s revitalization. Aaron earned a Master of Science in Social Policy from the University of Pennsylvania and a B.A. from Wesleyan University. When not in the office, Aaron enjoys spending time in the mountains climbing and hiking.

Jessica Valles is a community health worker at the Camden Coalition of Healthcare Providers. Jessica has been providing advocacy for patients and helping develop care management strategies since 2013. Prior to this role Jessica studied at the Star Technical Institute and specialized as a medical assistant.

She has her master’s degree in Social Work and has spent her entire career working with the complex, high-cost, high-needs patient population. In her free time she loves playing soccer, running, snowboarding, and camping.

Corey has been friends with the Coalition for years. He comes to us from Spectrum Health in Western Michigan, with extensive training in pain, addiction, and emergency room medicine. At Spectrum, Corey opened and led the Center for Integrative Medicine starting in 2008, which saw patients with major medical, hard to diagnose conditions such as psychiatric illness, chronic pain and/or addiction.

Dave Weinman joined the Coalition in June of 2014 as the senior program manager for Finance & Administration. Dawn Wiest: Associate Director, Action Research & Evaluation

Dawn came to CCHP in 2014 with a background in health, criminal justice, and social movement research. Before joining Camden Coalition, she worked as a senior research analyst at the American College of Physicians in Philadelphia and as Assistant Professor of Sociology at the University of Memphis in Tennessee.

Sierra Williams: Community Health Worker, Clinical Redesign Initiatives

Sierra is a community health worker on the Clinical Redesign Initiatives team. After several years working as a case manager in behavioral health, supportive housing and family services, Sierra joined the Camden Coalition to support its ACO initiatives to improve access to care for women of reproductive age, mothers and patients with significant social barriers to care in Camden. She graduated from Rowan University with a degree in Sociology.

Carter Wilson: Senior Program Manager, Clinical Redesign Initiatives

Carter Wilson serves as the senior program manager for Clinical Redesign Initiatives. In this role, he works with program leads to plan and launch projects and monitors the progress of organization-wide initiatives, including the Medicaid ACO Demonstration Project. Carter previously worked with the U.S. House of Representatives Foreign Affairs Subcommittee on Africa, Global Health and Human Rights as a Legislative Fellow. He has a Bachelor in Economics and a Master in Commerce and Economics.

Qiang Yang: Senior Program Manager, Data Science

Qiang Yang serves as the Senior Program Manager for Data Science in Strategy & Information department, where he will be mining data for key insights in support of the Camden Coalition’s mission and goals. He holds a M.S. in Statistics and a Ph.D. in Mathematics from Tulane University. In his spare time, Qiang loves reading books, listening to music and watching sports.
