Michelle Adyniec: Clinical Manager, Innovation Operations

Michelle is a Clinical Manager at Camden Coalition for the Population Health Initiatives Department. In this role, she works with individuals with medical and social complexities to identify barriers to care and assist in developing patient-centered care plans to improve medical needs of each client. Michelle is a registered nurse with extensive experience in the acute care setting and rural community health work in Guatemala.

Tina Ahmadinejad: Super-Utilizer Fellow

Tina is the “super-utilizer fellow” from the Crozer-Keystone- Camden Coalition partnership and is currently working with the Legal and External Affairs Department at the Camden Coalition. She received her B.A. in Neuroscience from Skidmore College in upstate New York, M.D. from Jefferson Medical College in Philadelphia, and completed her residency training in family
medicine at Crozer-Keystone in Delaware County, PA. Tina is passionate about preventive medicine and working with underserved patients in under-resourced communities.

Gladys Antelo: Program Manager, Relationship Management

Gladys Antelo is a Program Manager with the Cross-Site Learning and Workforce Development department. In this role, Gladys manages relationships for Student Hotspotting, technical assistance contracts and other educational programs the Camden Coalition offers. Previously she worked as a Nurse Care Coordinator with the Population Health Initiatives Department, working with patients in the community. Gladys is a Licensed Practical Nurse by training and graduated from Lincoln Technical Institute in 2012.

Mavis Asiedu-Frimpong: Director for National Initiatives

Mavis, along with the National Initiatives team, leads further development of the National Center for Complex Health and Social Needs. She has a background in law and health policy, having most recently served as Assistant Director of Policy and Planning for the San Francisco Health Department. She is a recent transplant to the Philadelphia-New Jersey area and spends her free time exploring the museums, restaurants, and parks in her new hometown.

Adria Boultin: Program Assistant, Information Systems & Data Security

Adria is a Program Assistant for Strategy & Information team; her role is implementing strategy and policies within the Camden Coalition. Adria is currently studying Computer Science at Camden County College and hopes to transfer to Temple University. In her free time, she loves to volunteer in community-based programs that prevent homelessness.

Sheila Brown: Chief Financial Officer & Senior Director for Administration

Sheila Brown is the Chief Financial Officer & Senior Director for Administration. Prior to joining the Coalition, she worked as the Controller for Merit Service Solutions. She spent 10 years working as a public accountant with the firm Withum Smith+Brown specializing in auditing and consulting for not for profit organizations. Six of those years were spent working primarily on the Healthcare Team. She was Director of Finance for Ambulatory Services at Virtua, and Controller for Silver Labs LLC,

Cortney Bruno: Research Assistant, Publication and Dissemination

Cortney is the Research Assistant for Publication and Dissemination in the Action Research and Evaluation Department. She completed her MSW at Temple University, with a concentration in Community & Policy work, and worked as a Research Assistant within the department. She also holds a BA in English and worked as an adult literacy instructor and case manager at Center for Literacy in Philadelphia before attending graduate school.

Whitney Buchmann: Senior Program Manager, Organizational Development & Effectiveness

Whitney is the Senior Program Manager for Organizational Development & Effectiveness. Her primary role is to support the development of processes and practices that keep the Camden Coalition's core values at the center. Prior to coming to the Camden Coalition Whitney spent six years as a Community Organizer with PICO National Network. Whitney holds a bachelor's degree in Spanish and International Studies, with a certificate in Conflict Resolution.

Laura Buckley: Senior Program Manager, Innovation Operations

Laura Buckley, M.S.W., L.S.W., leads a team of clinical specialists to design, iterate, and operationalize interventions that impact individuals with complex health and social needs, most notably overseeing the daily implementation of the Camden Coalition's Housing First Demonstration Project. Prior to joining the Coalition, Laura was a graduate teaching assistant at the University of Pennsylvania's School of Social Policy and Practice working in both Philadelphia and Kolkata, India. She received her master's degree in social work from the University of Pennsylvania with a clinical concentration and her Bachelor of Arts from New York University with a double major in history and journalism.

Jamie Cartagena: Staff Accountant, Accounting & Grants Administration

Jamie Cartagena supports both the Grant and Accounting Teams in his role as Staff Accountant. He has a degree in Accounting from Temple University, as well as a background in non-profit accounting and grants from his time with Nueva Esperanza. In his free time, Jamie likes to work on his car and garden.

Gayle Christiansen: Program Manager, Content & Experience Curation

Gayle creates curriculum and learning experiences from a human-centered design perspective at the Camden Coalition. She has experience teaching and designing instruction for elementary school through late career professionals. Gayle first came to Camden as a Camden Public Schools teacher. Gayle studied Sociology as an undergraduate at Kenyon College and holds a Master of City Planning degree from MIT. Gayle also serves as a part-time lecturer in the Urban Studies Department at Rutgers University – Camden.

Jessica Cordero: Senior Community Health Worker, Community Operations

Jessica Cordero is the medical assistant and senior community health worker for the Coalition’s Care Management intervention. With several years of experience managing a busy community-based primary care practice in Camden, she currently uses the same skills coordinating care for the project’s high needs patients. Additionally, she provides various levels of clinical assistance on site during home visits, as well as a liaison for patients who are Spanish-speaking. In serving patients who are medically and socially complex, she has become a dependable and trusted resource for these city residents. Jessica grew up in Camden and earned a degree as a Medical Assistant in New Jersey.

Kelly Craig: Chief Strategy & Information Officer

As one of the original ten members of the Camden Coalition, Kelly worked closely with Executive Director, Jeffrey Brenner, M.D., in developing the Coalition’s innovative care management model. This model engages patients with multiple co-morbidities that lead to high utilization of the local healthcare system. Kelly implemented the first managed care contract that lead to the Coalition's current Medicaid Accountable Care work. Additionally, she grew the Camden citywide Care Management Meetings that bring together stakeholders from three competing hospitals around common community barriers and shared patients. The structure she built over five years still meets monthly and serves as an anchor of collaboration within the Camden community.

Prior to coming to the Coalition, Kelly worked at Prevention Point Philadelphia, a harm reduction organization. There she coordinated the Street-side Health Project, a mobile medical clinic run solely on volunteer labor in partnership with local medical, nursing and pharmacy schools. Her passion drove her to establish the first low-threshold buprenorphine clinic in the area, giving access to individuals experiencing homelessness and opiod addiction. These programs are still thriving in Philadelphia. Currently, Kelly is focused on supporting the design, development, and expansion of a variety of population health initiatives taking place at the Coalition. Some current initiatives include overseeing our citywide 7-day pledge campaign, which focuses on getting patients into primary care within 7 days of a hospital visit. She is also working with community partners to advance the first regional Housing First program, a low-threshold model that provides housing and supportive services to the most medically fragile patients within Camden.

Amadly Cruz is a program manager for the Care Management Initiatives team. She received her B.A. in Psychology at Eastern University and plans to go back for her M.S.W. Born, raised and currently living in Camden, Amadly feels a deep connection with the community and wants to influence positive health to the people through the help of the Camden Coalition.

Meylin Davila: Program Manager, Clinical Redesign Initiatives

Meylin Davila is the Program Manager for the Clinical Redesign Initiatives team. She holds a B.A. in Sociology from Rowan University. In this role, Meylin will oversee the processes related to screening (or triaging) of data feeds from various clinical sources, primary care, and specialty appointment scheduling, and population-level patient outreach. Meylin is responsible for ensuring data collection from internal and external partners, managing the data flow for population-level programs, monitoring quality assurance dashboards, and maintain relationships with key ACO implementation partners, including primary care offices.

Natasha Dravid: Director, Clinical Redesign Initiatives

Natasha Dravid joined the Camden Coalition in 2013 and serves as Director of Clinical Redesign Initiatives. In this capacity, Natasha is responsible for the strategy and operations of the Camden Coalition Medicaid Accountable Care Organization and for managing contract relationships with Medicaid managed care organizations for the delivery of programming the serves individuals with complex health and social needs. Natasha holds an MBA from the Yale School of Management and a BA in English from Haverford College.

Abigail Fallen: Senior Program Manager, Health Information Exchange & Data Security

Abby Fallen is the senior program manager for the Camden Coalition Health Information Exchange (HIE). Abby oversees the program’s objectives while working with Camden’s area hospitals and primary care centers to develop effective training processes. Before joining the Coalition, Abby worked as an EMR Training Specialist for STI Computer Services, aiding and assisting various physician practices in transitioning their medical records from paper to an electronic format. Additionally, Abby has extensive knowledge of office workflows, clinical medicine, inpatient and outpatient documentation, and systems analysis. Abby is a graduate of Temple University and holds her Registered Health Information Administrator (RHIA) certification.

James Fisher: Program Manager, Analytics & Informatics

Jay is the program manager for the Information & Systems team at the Camden Coalition. He recently worked as a statistical research specialist for TennCare, Tennessee's Medicaid office. He also served for two years as a community health volunteer in Cameroon with the Peace Corps. Jay received his bachelor's degree in International Relations from the College of Wooster and his master's degree in Public Administration from the University of Georgia. Jay is interested in Medicaid policy, running when he can, and watching basketball.

Brian Fuss: Program Manager, Online Education

Brian oversees and coordinates the online education platform at the Camden Coalition. He has experience teaching, training, and developing courses on addiction, mental health, sexual health, and harm reduction. Brian has an M.P.A. and finishing his Ph.D. in Public Policy and Administration. In this limited free time, he loves to exercise, read nonfiction, review Grants.gov, and analyze public policies.

Brandon is a community health worker on the Innovation Operations team. He has worked in group homes for clients with developmental disabilities in a residential and day program setting. In his free time, he writes, arranges, produces, and record music.

Marizta Gomez: Program Assistant, Community Engagement

Maritza Gomez is the Program Assistant for Community Engagement. In her role, she coordinates the Coalition’s Faith in Prevention program which provides educational opportunities for faith-based organizations in Camden to incorporate nutrition and exercise into spiritual well being. Maritza also serves as support staff person in the Coalition’s community engagement efforts, specifically providing COACH model training throughout the city. She is passionate about serving as a liaison and advocate for the Spanish speaking community in Camden. Currently, Maritza attends Camden County College earning a degree in nursing.

Ebony Hailey: Nurse Care Coordinator, Community Operations

Ebony Hailey is a nurse care coordinator on the CMI team. Ebony works closely in the community, bridging the gap between patients and their healthcare providers. She was born and raised in the City of Camden and has a love for giving back to the community and mentoring. Prior to joining our team, Ebony was a Critical Care Technician in the Intensive Care Unit at Cooper Hospital University Center for seven years. She is currently pursuing her B.S.N.

Lauran Hardin: Senior Director, Cross-Continuum Transformation

Lauran Hardin recently joined the Camden Coalition and the National Center for Complex Health and Social Needs as the Senior Director of Education & Curriculum. In this role, she continues her past experience providing consulting, co-design and coaching in complex care transformation. Her special interests include the impact of trauma/loss on high-frequency healthcare access and the economic potential of stabilizing complex patients through retraining/redesigning existing resources in the healthcare system.

Margaret Hawthorne: Consultant

Margaret Hawthorne serves as the Consultant for the Camden Coalition, where she works with diverse stakeholder groups to design, evaluate, and transform operational processes and care delivery systems. Prior to joining the Coalition, Maggie worked as a Program Officer with the Institute of Medicine evaluating U.S. global HIV/AIDS programs for the US Congress, an epidemiologist managing the operations for two statewide HIV surveillance systems, and a Research Manager at Johns Hopkins University focused on developing tobacco control leadership capacity globally. Margaret holds a master’s degree in Public Health from Johns Hopkins University and is passionate about employing a mix of both qualitative and quantitative methods to understand, design, implement, and evolve relationships and systems that will lead to profound impact and change.

Audrey Hendricks: Enrollment Specialist, Hospital Operations

Audrey Hendricks is an enrollment specialist on the hospital operations team for Care Management Initiatives. In this role, she serves as the first point of contact for hospitalized patients who are eligible for Camden Coalition's Care Management Intervention, engaging and enrolling them into the intervention at the bedside. Audrey joined the Camden Coalition in 2012 as an AmeriCorps Health Coach. Following her year of service, she worked as a program assistant in the Cross-Site Learning program before beginning her current role in early 2015. She holds a B.A. in Anthropology from Haverford College.

Danielle is a Program Assistant for the Action Research and Evaluation team. In this role, she researches and evaluates projects within and across departments, as well as with external partners. Prior to joining the Coalition, Danielle earned a B.S. in Psychology from the Xavier University of Louisiana and a Master of Health Science degree with a concentration in Mental Health from the Johns Hopkins Bloomberg School of Public Health. She also worked as a Research Assistant for the Johns Hopkins Center for Adolescent Health.

Michaela Holmes: Program Manager, Organizational Development

Michaela Holmes is the Program Manager for Organizational Development. Prior to working at the Camden Coalition Michaela spent three years working to grow the cooperative economy in the Greater Philadelphia Area. Michaela holds a Master of Science in Organizational Change Management from the New School in New York, and a B.S. in Political Science from the University of Oregon. On her own time, she works towards social and economic justice, ponders the good life, appreciates nature, enjoys hikes with her dog Lou, and bakes bread.

Nate Hulfish: Project Manager, Consulting Operations

Nate is the Project Manager for Consulting Operations and is responsible for sustaining the technical assistance infrastructures within both the Cross-Site Learning and Workforce Development Department and the National Center for Complex Health and Social Needs. He earned his MBA with a concentration in Leadership from Eastern Mennonite University in April of 2017 and comes from a background in faith-based non-profit work, where he most recently worked as the Director of Operations for Circle of Hope. He is married with two children and spends his free time teaching/practicing yoga and watching Philadelphia sports.

Mark Humowiecki: General Counsel & Senior Director for National Initiatives

Mark Humowiecki leads the National Center for Complex Health and Social Needs, a new initiative of the Coalition that seeks to coalesce a new field of healthcare and build a national movement to improve outcomes for individuals with the most complex needs. Previously Mark led the Coalition's advocacy, communications, and governance efforts. Before joining the Coalition, Mark served as deputy executive director of the New York State Workers' Compensation Board and spent six years practicing employment and civil rights law. Mark earned his bachelors and law degrees from Yale University.

Jared Hunter: Program Assistant, National & Consumer Engagement

Jared Hunter is the Program Assistant for National & Consumer Engagement. In his role he supports the work in developing strategy, tactics, and rhetoric around community and consumer engagement on both the local and national levels; this includes building out the Consumer Scholars program through the Coalition's National Center for Complex Health and Social Needs and developing a new organization-wide strategic plan about how to effectively engage communities in Camden and across the country. Jared is currently pursuing his Master of Public Administration (MPA) degree with a concentration in Community Development at Rutgers University-Camden.

Stephanie Jean-Louis is the Program Assistant for Governance. In this role, she coordinates communication both externally, to key stakeholders and community partners, and internally. She received her Bachelor's degree in Biobehavioral Health from Penn State University. She is passionate about community organizing and enjoys spending time with family and friends.

Marsha Johnson: Chief Learning Officer & Clinical Director, Curriculum Integration

Marsha Johnson, Chief Learning Officer, leads teams that design and deliver technical assistance and educational programming to transform the field of complex care. Areas of focus in her prior work include behavioral health integration, trauma-informed care, and clinical redesign to advance the care of individuals with medical and social complexity. Marsha is a licensed clinical social worker and received her M.S.W. from Smith College.

Naadia Johnson: Program Assistant, Organizational Effectiveness & Performance

Naadia is a Rutgers University graduate with a B.A. in psychology and now fills the role of program assistant for Camden Coalition. As her favorite core value is servant leadership, her objective is to offer continued organization to better help our staff empower our patients and members. Future goals include continued education in social work with plans to provide services that give back to the city of Camden, her free time is spent reading, taking trips and scrap booking for memories.

Teray is the Program Assistant for Clinical Redesign Initiatives. In this role, Teray reviews eligibility to the Camden Coalition's clinical intervention and provides project management. Teray has a Bachelor's of Science in Healthcare Management from Towson University and is currently pursuing an MBA with a Management Information Systems specialization from Rowan University. She enjoys reading and writing fiction in her free time.

Matthew Kalamar: Associate Director, Grants and Development

Matthew is the Associate Director of Grants and Development. In this role, he spearheads the formulation and execution of the Coalition's fundraising strategy. Prior to joining the Coalition, Matthew worked as a Program Manager and Grants Coordinator on the International Programs team at Solidarite Sida, one of the main French HIV/AIDS organizations, based in Paris. Matthew holds an M.A. in Development Studies from the University of KwaZulu-Natal (Durban, South Africa) and a B.A. in International Relations and German Studies from the University of Pennsylvania.

Andrew Katz: Senior Program Manager, Care Management Initiatives

Andrew Katz is a senior program manager of Care Management Initiatives and focuses on operations and care delivery for one of the Coalition's outreach teams. Andrew joined the Coalition in 2011 as an AmeriCorps health coach and has since worked on CCHP projects like Diabetes Self Management Education, Integrated Diabetes Care Program, and practice transformation at two primary care clinics in the City of Camden. Prior to joining the Coalition, Andrew studied Psychology at McGill University.

Samuel Katz: Program Manager, Strategic Planning & Consultant

Samuel Katz is the Program Manager for Strategic Planning & Consultant at the Camden Coalition. In this role, he helps implement, monitor, and evaluate the Coalition's strategic initiatives. Previously, Samuel worked at the Mixed Methods Research Lab at the University of Pennsylvania. He has also worked as a medical-legal writer for a disability advocacy group, an EMT, and an assistant to a gourmet chef. Sam earned a B.A. from Wesleyan University and is currently pursuing an M.B.A. at Temple University.

Mingie Kang: Program Manager, Accountable Health Communities

Mingie is responsible for implementing the Accountable Health Communities (AHC) Model to systematically identify and address the health-related social needs of our patients. Prior to the Camden Coalition, she worked at Better Health for Northeast New York where she facilitated a multi-sector collaboration to implement a 5-year Medicaid redesign project called DSRIP. Mingie received her Masters of Public Health degree from Drexel University and her bachelor degrees in Biochemistry and Cognitive Science from University of Virginia.

Mary Kleva: Program Manager, Stakeholder Relations

Mary Kleva is the Program Manager for Stakeholder Relations within the Legal and External Affairs Department. She works on grant reporting and development, advocacy and policy initiatives, Camden Coalition board activities, and additional communications for the department. Prior to joining the Coalition, Mary was a research assistant at Public Health Management Corporation in Philadelphia. Mary graduated from the University of Pennsylvania with a B.A. in Health and Societies, concentrating in Health Policy and Law.

John Kosick is the Program Manager for the Strategy & Information team at the coalition. He provides user and network administration support for the Camden Coalition office as well as providing technical support to other non-profits in Camden and Philadelphia. Teagan Kuruna: Program Manager, Action Research & Evaluation

Teagan Kuruna serves as the Program Manager for the Action Research and Evaluation department, where she develops written content for journal articles, reports, and other publications. Prior to joining the Camden Coalition, she was a communications associate at the Center for Health Care Strategies and a communications intern for The Cancer Genome Atlas at the National Cancer Institute. Teagan holds a master of public health from Drexel University School of Public Health and a bachelor of arts from New College of Florida. In her spare time, Teagan organizes the Philadelphia Podcast Festival and hosts her own podcast.

Berly Laycox is the Communications Senior Program Manager for the Camden Coalition and the National Center for Complex Health and Social Needs. Berly has a B.A. in Communication from UMass Amherst and a B.F.A. in Print Media from Highlands University in Las Vegas, NM. In her spare time, she teaches coding to women in Camden and Philadelphia through Girl Develop It.

Vincent Leone: Program Manager, Instructional Design

Vince is a Program Manager with the Cross-Site Learning and Workforce Development department. In this role, he creates educational and operational infrastructure in order to codify and disseminate best practices while coordinating within the CSL & WD team, across the Coalition, and with the National Center. Previously, he worked as a secondary humanities educator at KIPP Philadelphia Schools.

Angelic Lopez: Accounting Manager, Accounting & Grants Administration

Angelic has joined the Camden Coalition as an Accounting Manager where she is responsible for managing the accounting team to ensure transactions are recorded accurately and timely to meet the organization's deadlines. Angelic received her Bachelor of Business Administration from Peirce College and her Master of Business Administration from Strayer University. Angelic likes to spend her free time with her family and friends - she enjoys helping others.

Christine McBride is the program manager of Information Systems & Technology. Christine's role is to plan and manage projects for new HIE interfaces and functionality, ensures compliance with the HIE Policies and Procedures through auditing and education, and assists in data security initiatives related to the approval, implementation and staff training of policies and procedures. Prior to joining the Coalition, she worked for a consulting firm as a marketing administrator and a project consultant. She holds a B.A. in Anthropology and a Master of Public Health from Yale University. Renee Murray: Associate Clinical Director, Care Management Initiatives

Renee is the Associate Clinical Director of Care Management Initiatives at the Camden Coalition.
She graduated from the University of Medicine and Dentistry of New Jersey with a B.S.N. and previously received a B.A. in Psychology from St. Joseph’s University. Her prior experiences include teaching at Camden Catholic High School instructing Psychology, Health, and Cultural Geography courses as well as being the Admissions Director. She also spent some time in the Dominican Republic setting up makeshift clinics to provide healthcare to the underserved locals. Most recently she practiced nursing on an Oncology/Hematology unit at Hahnemann University Hospital in Philadelphia. She joined the Coalition in 2012 as the RN Care Coordinator for the Pregnancy and Parenting Partners Program working with pregnant women throughout the city of Camden. Renee is now the RN Clinical Manager on Care Management Initiatives. CMI is a nationally recognized clinical intervention and research project seeking to improve health outcomes for complex, vulnerable patients through intensive community-based care management and care coordination activities.

Ramona Olivares has been instrumental in standing up patient outreach for the Accountable Care Organization (ACO) and the Camden Coalition Randomized Control Trial (RCT). In her current role, Ramona interfaces with all of the primary care practices in the ACO, assisting them in continuously improving their patient engagement strategies. In her spare time, Ramona likes to read, volunteer with her church, spend time with her family and friends, and visit seniors at nursing homes.

Latonya Oliver: Clinical Manager, Social Work for Innovation Operations

Latonya Oliver MSW, LSW is the Clinical Manager of Social Work for Innovation Operations. Latonya will work alongside the team's other Clinical Managers to oversee the non-medical needs of our patients in the community. Additionally, she will oversee the daily clinical workflow for our Housing First patients before move in and liaise with our on-the-ground Housing First Partners. Latonya graduated from Stockton University with a B.A. in Psychology and a minor in geriatrics. Latonya has graduated from Rutgers University- Camden campus with a Masters of Social Work and a concentration in clinical social work. During graduate school, Latonya also received a certification in aging/geriatrics. Previously, Latonya has worked with severe and persistently mentally ill clients within the Camden County area; during this time she was responsible for monitoring and using various interventions and techniques to reduce client readmissions to state and local psychiatric hospitals. Latonya also has work experience as an in-home therapist working with children who demonstrate emotional and behavioral issues due to exposure to trauma. Latonya is also pursuing her License in Clinical Social Work (LCSW) and will continue to build her knowledge around behavioral health. In her free time, Latonya enjoys traveling and spending time with her family and friends.

Alisha Patman serves as the Database Specialist for the Strategy and Information team. In this role, she assists with managing the Camden Coalition’s internal database. Hilary serves as the Program Manager for Advocacy and Engagement where she oversees the infrastructure and engagement strategies for the Good Care Collaborative, a project which convenes diverse stakeholders across New Jersey who is committed to improving the quality, outcomes, and efficiencies of the state’s Medicaid system. In this role, Hilary also contributes to the development of the Camden Coalition's state level, advocacy strategy. Janelle is the Program Assistant for Innovation Operations. Janelle will help design new interventions, like Camden RESET and Camden Delivers, that seek to improve health outcomes for complex, vulnerable patients through intensive care management and care coordination activities. Prior to joining the Coalition, Janelle researched the
neural correlates of skilled movement and neuromotor recovery after chronic stroke at the University of South Carolina where she completed a Master’s of Science degree in Exercise Science. Janelle also holds a Bachelor’s of Science degree in Kinesiology from the Pennsylvania State University.

Emily Rosenberg Spector serves as a Senior Program Manager in the Strategy and Information department at the Camden Coalition. She currently leads a regional collaborative comprised of five hospital systems, the goal of which is higher quality and better coordinated behavioral health care delivery for patients in South Jersey. Prior to joining the Coalition, Emily worked as a Manager for Research and Evaluation at the Montgomery County, Maryland, Department of Health and Human Services and at consulting firms in Baltimore and Washington, DC. Natassia Rozario oversees policy and advocacy efforts at the Coalition. She was a recipient of the Public Interest Public Service Scholarship at American University Washington College of Law and of the William J. Clinton Service Fellowship in India, and brings to the Coalition over ten years of experience and research devoted to improving the availability, accessibility, acceptability, and quality of healthcare for the underserved both in the US and abroad. She has a JD cum laude from American University Washington College of Law, a master's degree in Public Health from Johns Hopkins University, and a bachelor's degree from Columbia University, where she majored in political science. She received a Fulbright-Nehru research fellowship to study health and human rights in India and is a member of the New Jersey and New York Bar Associations.

David Scholnick is the Camden Coalition's Director of Communications. Before joining the Coalition, he spent four years with Everytown for Gun Safety where he contributed to life-saving policy changes as Pennsylvania state director and later as coordinator of the organization's Mayors Against Illegal Guns coalition. Dave enjoys playing with his son at DiSilvestro Playground where he serves on the Advisory Council and cheering for the Sixers through good times and bad.

Jeneen Skinner is a Clinical Manager for Camden Coalition's Population Health Initiatives. In her current role, she manages a care team and oversees a patient panel. Jeneen also assists patients with addressing barriers to care, creates care-plans, and reviews data to ensure that the metrics of the initiative are captured. Jeneen's experience includes working with vulnerable populations who have experienced homelessness, health/infection control, and sub-acute rehabilitation. Presently, she attends Wilmington University with plans to graduate in the Spring of 2018 with a Bachelor's of Science in Behavioral Science and a certification in Trauma Informed Approaches.

Tyson Smith is the Senior Program Manager of Qualitative Research. He currently works with incarcerated veterans and restorative justice programs in prisons. When he's not hanging out with his family, he tries his hand at gardening, cooking, ping pong, and running.

Jared Susco: Chief Operating Officer & Interim CEO

Jared grew up in Syracuse, NY but quickly fell in love with the Philadelphia/Camden area while an undergraduate at the University of Pennsylvania. He graduated from the Wharton School as a Bachelor of Science in economics with a concentration in accounting but decided to work in marketing strategy on Cap’n Crunch cereal and Quaker rice cakes for PepsiCo in Chicago. He then served as a rural economic development volunteer with the Peace Corps in Paraguay and subsequently pursued a certificate in Hebrew and Jewish Studies from the WUJS Institute in Israel. After returning to the US to earn a master’s degree in business administration with a concentration in decision and information analysis from the Goizueta Business School of Emory University, Jared moved back to the Philadelphia/Camden area to work in brand management with the Campbell Soup Company on several soup businesses.

Jared took a position as the director of operations and planning with the Undergraduate Division of the Wharton School, where his love began for strengthening operational structure and organizational capacity in the mission-based space. With successful stints as the comptroller and then chief financial officer of Penn Dental Medicine, he fully decided that operations and leadership in community-based healthcare was where he wanted to be. He is very excited to serve as the chief operating officer of Camden Coalition, where he brings business principles, like continuous improvement, systems thinking, and project planning, to the effective and efficient execution of the mission. Jared’s focus is largely internal, with emphasis on deepening structure, metrics, process, analysis, and planning for Camden Coalition’s existing programs and on supporting outcome- and performance-driven teams with mentoring, coaching, coordination, and integration. Jared and his husband live in the Society Hill area of Philadelphia, where they are slowly restoring the 204-year-old row home in which they live. Outside of work, Jared enjoys traveling, hiking, singing, supporting the arts, and spoiling his nieces and nephews.

Aaron Truchil: Director, Strategy & Analytics

Aaron Truchil serves as the Director of Strategy & Analytics at the Camden Coalition, where he oversees the organization’s data and research activities, including the Coalition's integrated data system that merges individual-level data from across sectors (health, criminal justice, and other social services), the Coalition's shared savings performance evaluations, and a variety of applied health services research initiatives aimed at developing an evidence base around community-based care and cost management initiatives. Prior to joining the Coalition, Aaron worked as a program manager at CamConnect, a non-profit data warehouse in Camden that analyzes and reports on data on the City of Camden’s revitalization. Aaron earned a Master of Science in Social Policy from the University of Pennsylvania and a B.A. from Wesleyan University. When not in the office, Aaron enjoys spending time in the mountains climbing and hiking.

Jessica Valles: Community Health Worker, Community Operations

Jessica Valles is a community health worker at the Camden Coalition of Healthcare Providers. In this role Jessica provides outreach and care coordination services to Camden patients with a history of hospital utilization for chronic disease related conditions. Jessica has been providing advocacy for patients and helping develop care management strategies since 2013. Prior to this role Jessica studied at the Star Technical Institute and specialized as a medical assistant.

Kimberly Verdecchio: Human Resource Coordinator, Human Resources & Talent Development

Kim is the Human Resource Coordinator for the Camden Coalition. Kim provides human resource support functions, assists with recruitment and on-boarding of new hires and interns, and provides benefits support to the Coalition. When she is not working, Kim enjoys spending time with her family, enjoying live music, and a good book.

Shelly Virva: Associate Clinical Director, Online Education

Shelly works for the National Center for Complex Health and Social Needs developing online curriculum for complex.care. She is also a member of the Cross-Site Learning team providing both online and on-site training as well as working with the teams to develop training. She has her master’s degree in Social Work and has spent her entire career working with the complex, high-cost, high-needs patient population. In her free time, she loves playing soccer, running, snowboarding, and camping.

Lauren is the Program Manager for Grant Writing and Reporting. Prior to joining the Coalition, Lauren was the Development Manager at the National Adoption Center in Philadelphia. She has an MPA from Widener University and a BA in Anthropology from Millersville University. Lauren loves animals, reading, spending time with her family.

Dawn Wiest: Director, Action Research & Evaluation

Dawn Wiest is the Director of Action Research and Evaluation for the Camden Coalition of Healthcare Providers and the National Center for Complex Health and Social Needs. Dawn has a Ph.D. from Stony Brook University and a B.A. in Sociology from Temple University. Prior to joining the Camden Coalition Dawn was a Senior Research Analyst at the American College of Physicians and an Assistant Professor of Sociology at the University of Memphis where her research focused on globalization and social movements.

Carter Wilson: Associate Director, Clinical Redesign Initiatives

Carter Wilson serves as the Associate Director for Clinical Redesign Initiatives. In this role, he works with program leads to plan and launch projects and monitors the progress of organization-wide initiatives, including the Medicaid ACO Demonstration Project. Carter previously worked with the U.S. House of Representatives Foreign Affairs Subcommittee on Africa, Global Health and Human Rights as a Legislative Fellow. He has a Bachelor in Economics and a Master in Commerce and Economics.

Katie Wood: Program Manager, Strategic Planning & Consulting

Katie is the Program Manager for Strategic Planning & Consulting. In this role, she supports the implementation of internal and external projects related to the Coalition's broader strategic plan. Previously, Katie worked as a project manager in the Philadelphia office of Schultz & Williams, a consulting firm specializing in the nonprofit sector. When she isn't working, Katie spends time with her family and friends, swims at her local Y, and continues her search for Philadelphia's finest baked goods.

Qiang Yang: Senior Program Manager, Action Research & Evaluation

Qiang Yang serves as the Senior Program Manager for Action Research & Evaluation in Strategy & Information department, where he will be mining data for key insights in support of the Camden Coalition’s mission and goals. He holds an M.S. in Statistics and a Ph.D. in Mathematics from Tulane University. In his spare time, Qiang loves reading books, listening to music and watching sports.

Tyler Yost: AmeriCorps VISTA, Strategy & Analytics

Tyler conducts research and analysis using integrated geospatial data from administrative and institutional sources. He attended Stockton twice, completing undergraduate degrees in computer science and psychology. Tyler then moved to Minneapolis to pursue graduate education at the University of Minnesota, where he earned a Ph.D. in psychology in 2016. While in grad school, his research focused on meta-analysis, publication bias, and questionable research practices.

Amy Yuen: Staff Writer, Communications

Amy Yuen is a Communications staff writer, creating the web and social media content to highlight patient stories and our projects and initiatives. A sociology major at the University of California, Davis, and a graduate of the MFA in Writing program at California College of the Arts, she managed communications for Stanford Graduate School of Education and worked in grantmaking to advance youth organizing and community arts as social change strategies. Amy is also a fiction writer, artist, and runner.
