Kelly Craig

Director of Care Management Initiatives

Kelly Craig is the Director of Care Management Initiatives, where she works to improve care coordination for medically and socially complex patients in Camden at a systems level.  She also works with county partners and state agencies to expand the efforts of community organizing in care delivery for high needs patient populations.  Kelly has an MSW and formerly worked with Prevention Point Philadelphia, where she worked directly with patients challenged with addictions, mental illness,  homelessness, and chronic illnesses.  She also helped run several street-side mobile clinics, which operated almost solely on volunteer labor.   Kelly is a graduate of the University of Central Florida and Temple University.
