Jason Turi

Clinical Manager

Jason Turi is the Clinical Manager for the Care Transitions Program, which embeds nurse coordinators and health coaches in primary care practices and outreaches to patients in hospitals and the community. The goal of this project is to improve care transitions and reduce hospital re-admission for medically and socially complex patients.  Jason is a public health nurse, labor and community organizer, and Spanish speaker. He has worked on various projects nationwide and abroad for social, economic and health justice.  His interests lie in promoting healthcare equity as well as harm reduction approaches to public health. He is a graduate of Union College, William Paterson University, and Boston University School of Public Health.
