Janet Parker

Nurse Care Coordinator

Janet Parker is the Nurse Care Coordinator for Project Dulce.  A Family Nurse Practitioner, she has worked extensively in pediatric ICU nursing and cardiovascular perfusion.  Using the Dulce curriculum, which follows the Chronic Care Model, and together with the project’s Peer Educator, Janet tracks patient outcomes, provides individualized education, supports the peer-to-peer component of the program, and works with primary care providers in coordinating the diabetes care and management of their patients.  Fluent in Spanish and having worked in a volunteer free clinic in Camden for over ten years, Janet has joined the Coalition to become more deeply immersed in community health practice.  She is a graduate of the College of Notre Dame, La Salle University, the College of New Jersey, and the University of Pennsylvania.
