The Social Worker will be an integral member of the Care Management Project’s multidisciplinary outreach team. Together with a nurse practitioner and bilingual medical assistant, the Social Worker will provide patient assessments (identify barriers to care, gather information for referrals, arrange home visits, etc…), provide short-term counseling to patients, help develop care management strategy for enrolled patients, and help create linkages for the various health and social needs of patients. The team works in the field in a variety of Camden settings, including patient homes, medical day centers, homeless shelters, and the ED/inpatient floors of each city hospital. This position will be supported by the Social Work Department of Cooper Hospital.
DUTIES and RESPONSIBILITIES
The primary responsibilities of this position include:
Provide initial intake of new patient referrals; assess patient’s social and health needs
Determine plan for care management; coordinate care plan with team’s Nurse Practitioner and medical assistant
Make appropriate referrals to county’s Board of Social Services and/or other social service agencies for social care; assist patients in making appointments and accessing other points of care
Determine patient’s financial needs; help patients gather personal IDs and other support documents to submit applications for government assistance; provide consistent follow-up with enrolled patients
Establish new partnerships with community resources in order to provide needed services for patients
Coordinate monthly Citywide Care Management Committee meetings; arrange logistics, plan guest speakers, prepare case presentations, etc…
Perform administrative tasks related to the project: patient scheduling, recording progress notes in patient charts (in an electronic health records system), etc…
Play a consistent and active role in identifying project inefficiencies and finding collaborative solutions to the problems
Other tasks as needed.
QUALIFICATIONS and REQUIREMENTS
BSW or MSW (preferred)
2-4 years of acute care or healthcare experience ideal
Strong familiarity with the Camden community/urban environment is preferred
Excellent organizational and interpersonal skills; must be able to keep organized progress notes on an electronic health records system; basic computing skills required
Demonstrated ability to work well with people of various ages, backgrounds, ethnicities, and life experiences
Ability to work independently and as part of a team; ability to multitask and utilize effective time management skills
Spanish language ability a plus
Licensure (LSW/CSW) in the State of New Jersey required
