Care Transitions Licensed Practical Nurse

Hilda Mateo is a licensed practical nurse for the Care Transitions Program, which embeds nurse coordinators and health coaches in primary care practices and outreaches to patients in hospitals and the community. The goal of this project is to improve care transitions and reduce hospital re-admission for medically and socially complex patients.  Hilda has worked in nursing homes throughout South Jersey as well as the Abigail House in Camden.  Through her experience there she has realized the need for improving care management and care coordination to improve the effectiveness of medicine.  She is a bilingual in English and Spanish and a recent graduate of Lincoln Technical Institute of Moorestown, New Jersey.
