The Camden Coalition of Healthcare Providers is a strategic initiative to improve the quality, capacity, and accessibility of the healthcare system for vulnerable populations in the City of Camden. In this live presentation, Dr. Jeff Brenner will present an overview of the Coalition and its work in Camden, including healthcare issues faced by the state and City, the Camden Health Database, and the Coalition’s initiatives to target specific patient populations. The presentation will conclude with time designated for open Q & A.

Since 2002 the Camden Coalition of Healthcare Providers has collected records on all admissions and emergency room visits from Camden’s three hospitals for all Camden city residents. Descriptive statistics and maps provide aggregate information to CCHP, the hospitals, payers and government organizations. The results identify opportunities to deliver better, less expensive healthcare. In this live presentation, Dr. Ernest Post and Aaron Truchil will present an overview of how CCHP has utilized data to implement effective healthcare programs. Topics discussed will include data acquisition, data cleaning and standardization, probabilistic linkage and geocoding, and statistical applications. The presentation will conclude with time designated for open Q & A.
