Project Manager

Nadia Ali is the Project Manager for the Camden Citywide Diabetes Collaborative, a multidisciplinary effort to improve diabetes care at the patient, practice and community level.

Jeffrey Brenner

Executive Director

Jeffrey Brenner is the Founder and Executive Director of the Coalition. He is a family physician and has practiced in Camden for eleven years as a front-line primary care provider for patients of all ages.

Jessica Cordero

Medical Assistant/Health Outreach Coordinator

Jessica Cordero is the Medical Assistant/Health Outreach Coordinator for the Coalition’s Care Management Project. She provides clinical assistance on site during home visits and works as a liaison for Spanish-speaking patients.

Kelly Craig

Director of Care Management Initiatives

Kelly Craig is the Director of Care Management Initiatives, where she works to improve care coordination for medically and socially complex patients in Camden at a systems level.

Megan Cunningham

Megan Cunningham supports the Coalition’s efforts in legislative and regulatory compliance, consulting contract management, grant writing, and communications.

Mark Difilippo

Project Manager

Mark Difilippo is the Project Manager for Patient Engagement Initiatives. His work focuses on bringing behavioral health, wellness, and peer support resources to primary care offices and hotspots throughout Camden.

Heidy Espada

Project Coordinator

Heidy Espada is an LPN and manages the New Jersey Medication Access and Compliance Program, where she helps qualified patients secure free medication through several pharmaceutical manufacturers’ patient assistance programs.

Kerry Gamble grew up in Moorestown, NJ, attended Moorestown High School and graduated from Dartmouth College. A geography and anthropology major, Kerry’s interests lie in social justice and preventative medicine.

Francine Grabowski

Lead Diabetes Educator

Francine Grabowski is the Lead Diabetes Educator for the Camden Citywide Diabetes Collaborative. She leads a community-based diabetes education program and coordinates with primary care practices.

Ken Gross serves as Director of Research and Evaluation where he oversees and manages the Coalition’s project evaluation and applied clinical research activities.

Assistant Executive Director

Sue Liu is the Assistant Executive Director of the Coalition. She supports and helps direct the Coalition’s projects and programming, develops new initiatives, works with partners across the city, and oversees organizational development for the Coalition.

Hilda Mateo is a licensed practical nurse for the Care Transitions Program, which embeds nurse coordinators and health coaches in primary care practices and outreaches to patients in hospitals and the community.

Intervention Specialist

Victor Murray is the Intervention Specialist for the Camden GPS Program, a hospital-based violence intervention program. Utilizing a care management approach, Victor works individually with Camden youth and young adults who are treated for injuries.

Ramona Olivares is the Project Assistant for the Citywide Diabetes Collaborative, where she assists the certified diabetes educators in all aspects of diabetes education implementation including referral and registration, in-class support, patient data collection, and information sharing with primary care physicians.

Janet Parker is the Nurse Care Coordinator for Project Dulce. Janet tracks patient outcomes, provides individualized education, supports the peer educator, and works with primary care providers in coordinating patients’ diabetes care and management.

Ernie Post is the Medical Director of Clinical Informatics, where he oversees the Coalition’s management of the Camden Health Database, a multi-year hospital claims database of all Camden hospitals.
